We offer you two easy ways to buy your health insurance.

1. By Phone.  If you prefer to speak to a representative, please call us toll free (866) 662-5433 and we’ll take your application over the phone.  Please have handy your Dr’s name and address, health card numbers for all proposed insureds, and either a credit card or your banking information.
2. Print and Mail.  You may also download the application, complete it at your convenience and send it to us by one of three methods:

• By Fax, toll free (866) 492-4419.
• By email, scan and forward to glenn@healthinsurancebrokers.ca
• By mail to: InsureCan Inc., 165 Peel Street Unit 2, New Hamburg, Ontario, N3A 1E7

Dental plans without catastrophic drug coverage – complete sections A,B,C,D,E on pages 1 and 2 plus the Applicant’s Declaration on page 4.
